Position: Community Health Officer (CHO)

Overview:
We are seeking a dedicated Community Health Officer (CHO) to strengthen primary healthcare delivery at the community level. The CHO will support preventive, promotive, and basic curative services, coordinate with healthcare teams, and help improve health outcomes through community engagement, screening, referral, and follow-up services.
Responsibilities
- Deliver essential primary healthcare services, including health promotion, disease prevention, and basic clinical care as per program protocols.
- Conduct community outreach activities, health awareness sessions, and behavior change communication initiatives.
- Perform screening and early identification for common conditions (e.g., hypertension, diabetes, maternal and child health risks) and ensure appropriate referrals.
- Support maternal, newborn, child, and adolescent health services, including antenatal/postnatal counseling and immunization coordination.
- Maintain accurate patient records, registers, and periodic reports; ensure timely data entry and documentation.
- Coordinate with medical officers, ANMs, ASHAs, and other frontline workers to ensure continuity of care and follow-up.
- Assist in planning and implementing public health campaigns and national health programs at the community level.
- Ensure compliance with clinical guidelines, infection prevention practices, and quality standards.
- Participate in trainings, reviews, and performance evaluations as required by the program.
- Support community-level documentation and verification processes when required by the organization/department.
Requirements
- Education/Qualification: Relevant qualification for Community Health Officer role as per applicable government/program norms (e.g., nursing/health sciences background with required certification/bridge training where applicable).
- Knowledge of primary healthcare delivery, community health principles, and public health programs.
- Ability to work in field/community settings and coordinate with diverse stakeholders.
- Strong communication skills in local language(s); ability to conduct community sessions effectively.
- Basic computer literacy for documentation, reporting, and data management.
- Integrity, empathy, and a patient-centered approach to care.
- Willingness to be posted as per service requirements within Bihar.
