Responsibilities

  • Deliver primary healthcare services including assessment, basic treatment, and follow-up care in accordance with NHM protocols.
  • Support implementation of national and state health programs (maternal and child health, immunization, communicable and non-communicable disease programs, etc.).
  • Conduct community outreach, health education, and awareness activities to promote preventive health practices.
  • Identify, counsel, and refer patients requiring higher-level care to appropriate facilities.
  • Maintain accurate records, registers, and reports as required for public health monitoring and program evaluation.
  • Coordinate with ASHAs, ANMs, and other field staff to ensure continuity of care and effective community engagement.
  • Support screening initiatives (e.g., hypertension, diabetes) and ensure timely follow-up and adherence.
  • Ensure ethical practice, patient confidentiality, and adherence to applicable clinical and administrative guidelines.

Requirements

  • Education: B.Sc. Nursing OR Integrated curriculum of Certificate in Community Health for Nurses (CCHN).
  • Registration: Valid registration with Indian Nursing Council and/or State Nursing Council (as applicable).
  • Age: Up to 35 years as on 07 February 2024 (relaxation as per government rules).
  • Skills: Community health orientation, patient counseling, basic clinical assessment, documentation, and coordination with public health teams.
  • Desirable: Relevant experience/knowledge as per official notification (candidates should review the notification for specifics).
